In the last few years, MSF OCG has developed an Operational Policy with the ambition to increase and improve the quantity and quality of secondary health care structures (or inpatient care). This recognition has prompted the organization to take a closer look at the challenges, lessons and accomplishments in terms of hospital management to develop strategies that will enable the organization to successfully set up, govern, implement and exit inpatient projects in all types of contexts.

During the 2014-2015 Ebola virus disease outbreak in West Africa, MSF built a number of Ebola Treatment Centres (ETCs). MSF set up centres in the three countries at different moments and with different MSF Operational Sections, which resulted in a heterogenic collection of solutions. This review was conducted jointly for all MSF operational centers (OCs). It was commissioned by OCA and was conducted as part of the OCB Ebola review.

OCB initiated the Kibera project at a time when there was a lack of access to affordable HIV/TB health care and succeeded in providing HIV drugs in Kenya, against all odds. The initial decision to intervene in Kibera was based on need and while the needs remain massive, OCB is no longer alone in providing HIV and TB treatment, hence the opportunity to handover the responsibilities of the health facilities to the Nairobi County to ensure that the population is not abandoned and that the quality care and treatment is continued.

In 2010 the operational prospects for OCB (strategic operational plan) outlined the broad objectives for the coming three years and placed renewed attention on key medical areas including surgery and emergency and acute medicine, bringing about increased investment in two hospital programs where OCB focussed on providing emergency surgical care in third level facilities in Tabarre and Kunduz.

Following the earthquake in 2010 and the emergency response that ensued, MSF-OCG took the decision to invest in a hospital in the Leogane area that was for several years the biggest hospital managed by MSF-OCG. Given that for the last three years this was the only MSF-OCG project in the country, that the capital is located only two hours from the hospital and that there is a strategic ambition within MSF-OCG to review mission set-ups, OCG and the field teams determined that 2013 was an opportune time to review the Haiti mission.

The MSF-OCG handover process of the nutritional programme in Zinder was evaluated with the objective to assess best practices, challenges and lessons learnt of the handover/integration process of the Zinder CRENI to the hospital, with the main purpose to improve handover processes in such contexts as well as decision-making in future projects. The evaluation has confirmed the pertinence of MSF in Zinder since 2005 and has contributed to the reduction of the infant mortality rate.

The ambition to improve hospital care outcomes is a high priority in the MSF movement today. Over the past few years evaluations of hospital projects have been carried out by all the operational MSF sections at various points in time. The aim of this meta-review of 14 hospital evaluations is to highlight lessons learned and identify some of the major themes, which emerge for discussion.

After the 12 January 2010 earthquake in Port-au-Prince, 4 MSF sections (OCG, OCA, OCP and OCB) decided to build container hospitals to respond to the health care needs (including Léogâne). All of these prefabricated structures, though varying in quality, were unique in that they were partially or completely built from modular containers (with different architectural designs). Construction was done by subcontractors, MSF teams trained in construction, or a combination of the two.
