Four years after the 2005 Niger crisis, many things have changed in the nutrition field. This cahier aims at considering this evolution and exploring new possibilities for action for MSF: how can these changes  get MSF to reconsider its own goals and move its areas of intervention? How can new knowledge and the experience gained by our teams since the crisis in Niger lead to new operational ambitions?

In the beginning of 2006, OCB opened a hospital project in Bor, in Jonglei state, South Sudan. This was a little over a year after the CPA (the Comprehensive Peace Agreement) was signed between SPLA and GoS The project soon became one of the highest resources consuming project in an unstable environment. In mid 2008 the decision was taken to close the project. OCB considered that they were unable to manage the hospital. It was not thought that the project in the current format was sustainable security wise, given a number of factors.

In June 2009, after three years of the project by MSF-CH (2005 to 2008) in Dabola, Guinea, a final evaluation was conducted with the objective to (i) document the project’s strategies and (ii) undertake a critical analysisof the project regarding access to health care, perception, sustainability and impact.

Médecins Sans Frontières has been running HIV/AIDS treatment programmes since 2001. As national systems scaled up capacity, MSF began to hand over HIV projects in 2005. Variable experience with handover and internal debates about MSF responsibility to patients led OCG to propose an evaluation of the handover process, to learn from experience and inform policy and practice. A qualitative review was carried out through interviews, case studies in Cameroon and Laos, and review of other vertical and integrated HIV projects.

The evaluation unit in Vienna (MSF-Austria) was requested to analyse the different nutritional strategies implemented in Marial Lou over the past years (2003 – 2006) in terms of appropriateness, effectiveness and efficiency, and, to identify recommendations for future similar contexts. After presenting the first results, the evaluator was requested to extend the analysis of outcome data to those of the Niger nutrition program 2005 and 2006, for better comparison of results.

According to the terms of reference, the aim was less to ‘evaluate’ our intervention than to subject it to a critical examination which would enable us to “identify our weaknesses and the ways in which they can be corrected”. MSF-CH did rehabilitate the Mundari hospital in Kajo Keji County (KKC) and did support the existing OPD from September 1997 onward. Environmental- and political factors and an estimated high HIV prevalence did lead to the decision to implement the first time an integrated HIV/AIDS component (2005). Towards the end of the project it was decided that the handover process, as well as the HIV integrated approach and the capacity build during the last 9 years of presence in KKC should be evaluated.

This evaluation concerns the Natural Disaster Emergency Preparedness & Response Programme (NDPrep) executed by MSF-H in Bangladesh and the resulting interventions related to the floods in the period July - November 1998. A field visit to Bangladesh was made from 30 November - 10 December.
